Dear Parent or Guardian:
With the start of a new academic and athletic year, we want to inform you of insurance coverage for athletes at Mary Baldwin College. The policy we provide offers secondary coverage to the athlete’s primary insurance. A sample of how this works follows.
If your daughter “Jane”, a Mary Baldwin College athlete, is injured during soccer practice with a severe ankle sprain, then the Head Athletic Trainer may refer her to a physician or emergency room to rule out fracture. When “Jane” is sent to a physician or emergency room, the primary insurance listed will be her health insurance carried on her by parents, guardian, or herself. If “Jane” has a co-pay, then she will be responsible for that co-pay, but she can file a claim with our insurance carrier, Commercial Travelers, for reimbursement. If any bills are sent that are not covered by her primary insurance, then these itemized bills may be submitted with an explanation of benefits from the primary insurance carrier to our athletic insurance carrier, Commercial Travelers , for consideration of payment. With this in mind, a claims form must be completed and signed by the athlete (or guardian if not adult) and the Head Athletic Trainer before being sent to our insurance carrier, Commercial Travelers Mutual Insurance Company. This form will be available to the athlete or parents by contacting the Head Athletic Trainer at Mary Baldwin College.
In addition, receipts from co-pays, explanation of benefits from the primary insurance carrier, itemized bills from the medical provider, and physician diagnosis form must be sent to Commercial Travelers before any payment or reimbursement will be made. Any pre-existing injuries will not be covered, unless the athlete has been granted written medical clearance or release from a licensed physician. This medical clearance must be on file with the Head Athletic Trainer. This policy is intended to defray the costs of medical expenses that may occur with injuries from athletic participation; it is not absolute coverage for all medical expenses due to injury.
It will be the athlete’s and/or parent/guardian’s responsibility for coordinating explanation of benefits, receipts of payments, itemized bills, completed claim form and other required paperwork to be forwarded to Commercial Travelers Mutual.
